Quinault Community Health Primary Care is happy to welcome Monica Hallak, ARNP, Elizabeth Pennington, PNP, and Jean Ebersole, ARNP. Please call our office to begin the New Patient enrollment process.
 

Current Patients

If you are an established patient, now you can use this website for requesting a new appointment, request a refill, communicate with the office staff, pay bills, etc at your convinence. If this is an emergency please call 911 or visit the nearest emergency room.
